Adult Individual Plan $275 a year
Children’s Plan (under 13 years old): $250 a year
This is an annual reduced fee dental membership plan that allows individuals and families to receive quality dental care from Sunlight Dental Group for a percentage off the office’s usual, customary and reasonable (UCR) fees. This is not an insurance plan, no claims are filed, and no payments are made to any other healthcare providers. Sunlight Dental Group is not a licensed insurer, health maintenance organization, or other underwriter of health services. This plan may not be combined with any other offers, discounts, insurance, discount plans or advertisements.
Membership
Once the annual fee is paid, you will be entitled to the dental benefit membership program for 12 consecutive months. You have the right to cancel at any time, by submitting a written request, however this plan is non-refundable. Payment of full fee for the entire year is expected at time of enrollment.
The Membership Benefits Includes:
- (2) Healthy mouth cleanings *in the absence of periodontal diseases
- (2) Exams per year
- (2) Fluoride treatments at time of cleaning visits, for both children and adult
- All necessary x-rays at each visit
- Priority same day appointment for emergencies
- PLUS: 10% Discount on all dental services
